Personal
Care Services
It is common for seniors and disabled adults to have difficulty with their self-care activities or activities of daily living (ADLs). Our caregivers can assist your loved ones with their personal care and grooming activities including dressing, bathing, oral hygiene, toileting, and ambulation.
Self
Directed Care
We can help you keep all of the care your loved one receives under control by coordinating medication reminders and doctor visits and working with family caregivers.
Light Housekeeping
If you need help in the house with daily chores such as washing dishes, laundry, cleaning your bathroom, and/or ensuring that your space remains tidy we offer light housekeeping service.
Meal Prep & Home Helper
Many seniors and disabled adults cannot cook for themselves. Our caregivers can provide meal preparation to ensure that your loved ones get good, regular meals.
Transportation
When seniors give up driving, they give up a little bit of their freedom. Our caregivers can help your loved ones with their essential transportation needs.
Companionship
When your loved one is in our care, they will receive more than just a caregiver. Our caregivers will establish a personal relationship with your loved ones to be their companion as well.

CASE MANAGEMENT REFERRAL
CarePartners Home care is concerned about the total well-being of our consumers. We have a variety of resources that we'd like to ensure you have access to. We have an in-house dedicated case manager to assist with Home & Community Based Services. These services may include: may include, home healthcare, physical or speech therapy, meal delivery or food delivery, personal care (like dressing, bathing, eating, etc.), house cleaning, and other services.
